You are a nurse working in the Med-Surg unit of RMC hospital and are assigned to the care of Mr Brooks, a 72 year old male who has just undergone surgery. Mr Brooks has been bed-ridden for some time and nurses in the ICU inform you that he has the beginnings of a decubitus ulcer on his left buttocks. The nurse reports that the skin remains intact, reddened area is approximately the size of a quarter and is warm to touch. As Mr Brooks will be confined to his bed for at least another week you want to take precautions to make sure the ulcer does not progress further.

In order to correctly assess Mr. Brooks and prevent his decubitus ulcer from progressing, there are a few things you need to know.
When you are finished with this web quest, you will be able to:
1.. Define decubitus ulcer and list common names.
2. Explain how decubitus ulcers form and what are common sites for these ulcers.
3. Discuss what conditions/ diseases may increase a person’s risk of developing a decubitus ulcer.
4. Describe why preventing pressure ulcers is so important and various methods of prevention.
5. Identify and describe the stages of decubitus ulcers. Include pictures of each stage.
Based on what you have researched...
1. What stage decubitus ulcer does Mr. Brooks have?
2. Find a picture of what Mr. Brook's ulcer might look like.
3. What precautions will you take to ensure the ulcer does not progress any further?
4. During your daily assessment of the ulcer, what type findings would lead you to believe that the ulcer has progressed to the next stage?
Process
- You will research, via the internet, questions 1-5 under the "task" tab. (what will you learn)
- Complete the tasks in order.
- Title of each page/slide should be the question you are answering.
- You may present this information any way you like. BE CREATIVE. Don't bore me to tears. Only stipulation is that it must be in your google drive so you can attach it to the assignment in classroom.
- You must include pictures of all stages in #5. You can do each stage on a seperate page/slide just be sure to label it the correct stage.
- Information must be in complete sentences and in paragraph form. NO BULLETS.
- Do not simply put question 1: and then type an answer. I want this to be in narrative form. Explain in your OWN words.
- You must include a reference page with links to the websites you use. Title this page "References"
For the final report you will:
- Go under the "task" tab and "based on what you have researched"
- After completing all of your tasks, you will complete a final report on Mr. Brooks to give the oncoming nurse an update on his condition from admission assessment to current changes.
- Write in complete sentences and paragraph form. Title of this should be " Final report on Mr. Brooks"
